The purpose of this job description is to establish and define the authority, accountability, reporting relationships, responsibilities, and measures of performance of the Care Coordinator. 

Summary: The primary focus of the Care Coordinator is to help make life more rewarding for our clients – children and adults with Intellectual Developmental Disabilities and Mental Health Diagnoses – by coordinating the delivery of high quality patient care support and therapeutic services for their personal caseload. Accomplishes these results by effectively managing referrals, establishing quality interdisciplinary client care plans, ensuring resources are engaged effectively to meet client needs, orienting/educating staff-clients-client’s families about company services and procedures, supporting accurate and complete documentation and database management, and generating timely and accurate invoices in support of the Company’s delivery of services to our target population of children and adults with disabilities and mental health diagnoses The Care Coordinator reports to the CEO. 

Primary Responsibilities 

Client Care Plan 
  • Maintains communication with clients’ families as needed 
  • Coordinates information and client care requirements with case managers, program directors, therapists, and field staff as needed via email, telephone, etc. 
  • Develops interdisciplinary care plan and other case management tools 
  • Provides support and resources for field staff 
  • Complies with all state and federal privacy laws and requirements 
  • Maintains quality assurance 

Invoicing/Documentation Processing 
  • Reviews therapists’ documentation for completeness and correctness, and returns incorrect and/or incomplete documentation to the therapist with instructions for edits 
  • Sends completed documentation (quarterly reports, yearly reports, evaluations, missed visit reports, etc.) to partnering agencies 
  • Processes therapists’ session documentation for billing 
  • Maintains organization of documentation for billing
  • Completes billing and invoicing procedures for partnering agencies 
  • Follows up on invoices submitted to ensure proper receipt and processing. 
  • Conducts Accounts Receivable activities to ensure complete processing and payment of all documentation.
Referral Management 
  • Handles communication with agencies regarding client referrals 
  • Resolves issues that could affect smooth care progression 
  • Orients and educates staff, clients, and clients' families about company services and procedures 

Client/Service Documentation & Database
  • Responsible for maintaining and managing the internal client information 
  • Maintains patient care database by entering new information as it becomes available; verifying findings and reports; backing up data 
  • Respects clients by recognizing their rights and maintaining confidentiality 
Communication 
  • Demonstrates positive, helpful attitude in all communication & work activity 
  • Phone & Email - Generate, receive, and respond to email from/to the office, fellow employees, management team members, partnering agencies and clients, as needed. 
  • Meetings – Attend, contribute and provide leadership to team meetings for goal setting, planning, and organization for Simple Steps. 
  • Support clients, client family members, agency representatives and team members for all business and client care issues by effectively educating them about our services and procedures. 
  • Support the coordination of all Company departments and functions for enhancing teamwork and quality results. 
  • Receives redirection and corrective action for responding to inaccuracies or deficiencies in the work process with balance and receptivity; takes appropriate corrective action without resistance 
  • Supports customer and team satisfaction, supporting an environment of high quality service and mutual respect.

Miscellaneous 
  • Perform administrative duties as needed to complete documentation of work 
  • Conducts self in professional manner at all times

  • Is efficient with time, materials, and methods to ensure efficient Company operations 
  • Other duties as assigned 
JOB REQUIREMENTS: 
 
EDUCATION and/or EXPERIENCE 
Associate Degree and/or Bachelor Degree plus a minimum of one year of work experience, preferably in the health care or social services. Experience with individuals with disabilities is preferred. 
 
LANGUAGE SKILLS 
Excellent Telecommunication, organization, leadership, team, and customer service skills. 
Ability to read, analyze and interpret standard business documents.  Ability to interact effectively to inquiries or complaints from customers or other team members regarding all business issues & operating concerns.  Ability to write effective business emails. 
 
MATHEMATICAL SKILLS 
Ability to understand and use basic math. 
 
COMPUTER SKILLS
Must be computer literate with one to three years of experience using computers in a business environment.  Must be proficient in using Microsoft Office, Google Suite, and Adobe Acrobat software packages.
 
REASONING ABILITY
Strong attention to detail
 
Ability to define problems, interpret data, establish facts and draw valid conclusions – to solve problems effectively.  Ability to understand a variety of instructions in mathematical or diagram form and deal with abstract and concrete variables. Ability to manage time effectively and work independently.
 

CERTIFICATES, LICENSES, REGISTRATIONS
Background check: must be able and willing to pass a criminal background check and drug screen test.

PHYSICAL DEMANDS
While performing the duties of this job, the employee is required to walk, crouch, climb stairs, talk and hear.  The employee is required to lift/carry objects of up to 25 pounds.  The employee is also required to have average dexterity to use arms, hands and fingers to handle, feel and reach.  Specific vision abilities include close vision, distance vision, depth perception and ability to adjust focus.


WORK ENVIRONMENT
While performing the duties of this job, the employee sits for extended periods of time at a desk in front of a computer.


WORK HOURS:
Forty (40) hours per week with occasional extra hours, if needed.  Actual daily hours will be defined by the work demands as assigned and will normally range between 8:00 A.M. and 5:30 P.M.  

 

This is a remote position.

Compensation: $37,250.00 per year
